What Is Hospice Care?

Hospice is designed to bring comfort and support to people with terminal illness, and their families. When a patient elects to receive hospice care it means that she, or he, has decided to forgo any further aggressive, curative treatments and, instead, focus on comfort care and quality of life. We want to keep our patients out of the hospital and at home where things are familiar and life is more comfortable.

Hospice Care Is Provided By A Team of Professionals

led by the nurse case manager, in conjunction with the patient's PCP and the hospice Medical Director.
The team also includes nurse's aides, medical social workers, and bereavement counselors. Physical therapists, occupational therapists, speech therapists, certified dieticians/nutritionists and volunteers are also available, as needed.

Medicare Hospice Benefit Provides Four Levels Of Hospice Care

Patients may be admitted at any level of care. These levels of care are designed specifically to meet the changing needs of hospice patients as symptoms and circumstances necessitate.

Home Health Aide Services

Hospice aides provide personal care services to ensure a clean and comfortable environment for our patients and their families.

Therapy (ST / PT / OT )

Eligible hospice patients may receive up to three therapy visits (physical, occupational, or speech therapy) when needed to help palliate symptoms, improve safety, maintain function, and enhance quality of life as part of their individualized plan of care.

Unique Therapies

Our Unique Therapies program includes: massage therapy a valuable part of hospice care, that helps ease pain, reduce muscle tension, promote relaxation, and enhance overall comfort. Music therapy provided by a licensed music therapist, offers meaningful moments of connection, reduces anxiety, promotes relaxation, and brings comfort to patients and their loved ones. We also offer pet therapy.

Volunteers

Our hospice volunteers are an invaluable part of our care team, providing compassionate non-medical, emotional, and practical support to patients and their families.

Bereavement Counseling

Our Bereavement Counseling program provides helpful resources, private and group counseling, and compassionate support to family members and loved ones as they navigate the grieving process.
